Service Placement Contract

When you start volunteering, you will be required to fill out a contract and have your supervisor at the organization you’re volunteering with sign it for you. You are required to turn in a contract by midterm in order to pass the course, so we recommend bringing it with you to your first volunteer shift or orientation meeting and have it signed then. We also recommend letting your supervisor know that they will be asked to fill out an evaluation at the end of the semester based on your performance.

Short-term Placement Contract

The Short-term Placement contract is designed specifically for placements where you are volunteering for 10 hours or less over 3 shifts or less. If you are going to be volunteering for more time or more shifts than that, please use the standard contract above. This contract serves as both the contract and the timesheet for this placement, and you do not need to fill out a separate timesheet for a short-term placement. All volunteer shifts on the Short-Term Placement contract must be initialed by the supervisor and the form must be signed by both the student and the supervisor before being handed in. 

Timesheet

You will also be required to keep track of your hours using a timesheet. You do not need to use this template, but it is provided for your benefit, since it’s formatted to automatically calculate your hours. Your supervisor will be asked to confirm the hours you served. Please fill out a separate timesheet for each placement where you served (unless you have turned in a short-term contract for that placement.) 

Important: In order to use this timesheet, please go to File > Make a Copy, which will create your own copy in your personal Google Drive. You will not be able to edit the original file.
